Job Description Summary

The Nurse Practitioner (NP) with PDM's Personalized Care Clinic (PCC), which includes the Care Center for Individuals with Disabilities, will be a key team member supporting the fourth-year dental students and serve various stakeholders including students, faculty, staff, and patients.

Job Description

REQUIREMENTS:

Nurse Practitioners with Penn PCC are responsible for supervising, supporting, developing and assisting pre-doctoral and post-doctoral dental students with medical history review and risk assessment, ordering and understanding laboratory tests and results, physician referral and contact, nutritional counseling, taking and understanding vital signs, and proper documentation and tracking of medical information in the electronic health record. Comfort with patients including but not limited to individuals with complex medical concerns; intellectual and physical disabilities; behavioral, emotional and cognitive limitations is a must. The person must have a passion for the proper care and well-being of patients and an ability to facilitate medical care for individuals that report to PDM without a medical home.
  • MSN or DNP, certified as a family or Adult Gerontology Primary Care NP.
  • Current Pennsylvania nurse practitioner license.
  • Knowledge of dental terminology, procedures, and an understanding of the relationship between oral health and overall health a plus.
  • Familiarity with federal and Pennsylvania's informed consent and guardianship laws and state policies regarding individuals with disabilities.
  • Ability to communicate in fluent English orally, in writing, on computer or via other communication devices.
  • Ability to communicate complex information effectively.
  • Ability to multi-task, set priorities and follow through with general supervision.
  • Maintain a professional appearance.
  • Proficiency in EHR charting systems and computer-based data management programs and information systems.
  • Employment clearance through the State of PA to work with children: as determined through Pennsylvania Criminal Background Check, Pennsylvania Child Abuse Clearance and FBI Finger Printing Database is required.


POSITION EXPECTATIONS
  • Work with, supervise, and support students in medical history taking and medical risk assessment. Work with students to ensure medical records are accurate and complete.
  • Work with, supervise, and support students in the taking of vital signs and limited physical assessment of patients in terms of medical history and review of systems.
  • Work with dental students in developing patient behavior support plans.
  • Work with dental team to obtain consents for care, treatment plans, and other consents as needed.
  • Work with students and dental care team to identify need for and facilitate referral to community based providers. ( non-dental providers) create and maintain referral path.
  • Assist with timely information sharing between healthcare providers and PDM. Ensure that faculty and students are aware of information received from other providers.
  • Apply APRN skills and knowledge to educate and motivate patients by demonstrating effective communication to bridge the gap between overall health and oral health.
  • Apply APRN skills and knowledge to facilitate inter-professional communication and treatment planning tailored to best meet the overall health of individuals while addressing their oral health needs, including nutritional counseling and tobacco cessation; with providing post-operative instructions that are consistent with the medical risk assessment; ordering, interpreting, recording, and monitoring diagnostic tests. Assist students in relating test results and medical implications to patients and/or caregivers.
  • Other duties as assigned, working with the community one health hygienist, supervise and facilitate tele-denistry appointments.

  • To date, clinical hours are Monday through Friday 7:30AM - 5:30PM; flexibility in scheduling is required for all positions (9 hours/day).
  • Contributes to team effort by accomplishing and supporting as assigned.

Salary offers are made based on the candidate's qualifications, experience, skills, and education as they directly relate to the requirements of the position, as well as internal and market factors and grade profile.
